Important Graduate School Forms
APPOINTMENT OF ADVISORY COMMITTEE
The advisory committee consists of four members: the advisory committee chair, two faculty members from the Department of Kinesiology & Nutrition Sciences, and a representative from another department. The advisory committee chair must have full graduate faculty status. The remaining advisory committee members must hold full or associate graduate faculty status. Students should select this committee after completing 12-16 credit hours and prior to establishing and submitting their proposed degree program.

These forms should be completed, in consultation with your advisory committee chair, before completing 16 credit hours or before completion of your second semester.
PROSPECTUS APPROVAL FORM
ORAL DEFENSE RESULTS (THESIS/PROFESSIONAL PAPER) The due
date for this form is the same as the deadline to submit a
thesis or dissertation to the Graduate College - or a minimum
of 2 weeks before the end of instruction of the term in which
graduation is anticipated.
GRADUATION APPLICATION:
The deadline date is available on the Graduate College website and is
approximately 3 weeks after the first day of instruction of any given semester
